At Christian County Foot and Ankle Center, every patient who comes through our door deserves to be treated with the utmost care and attention.

We understand that visiting a new doctor for the first time can be a little anxiety-inducing, which is why we wish to provide as much information as you need in a place you can easily access it. The tips and information here will also help you through our initial registration process smoothly, and get you to the important part of addressing your foot and ankle concerns all that much sooner.

Patient Forms

We require all patients to fill out a registration form before their first appointment. This gives us the information we must have before we are able to provide any services.

Our form also details our office policies, including rescheduling and records requests.

Once your appointment is scheduled please print and complete the new patient packet. The form can be faxed or mailed to the office before your appointment date.

INSURANCE INFORMATION

We accept most insurance. Please call the office for details.

WHAT SHOULD YOU BRING TO YOUR FIRST APPOINTMENT?

A patient’s needs may vary from person to person, but here are a few things you should consider bringing with you the first time you see us:

  • Your insurance card or authorization.
  • A pair of shoes that you wear frequently (you can even wear them in!).
  • A list of medications and supplements you are currently taking.
  • A list of any allergies you may have.
  • A list of medical conditions you currently have, as well as prior surgeries or hospitalizations (anything that wasn’t presented on your registration form).
  • Any medical records, imaging test results (e.g. X-rays), or lab results that may be pertinent to your concerns.
  • A list of any questions you wish to ask. (And we really do encourage you to write these down. It’s easy to forget some of the questions you had during your appointment!)
